Indoor Class Indemnity Form.

Before attending our indoor classes, this form must be filled out on the day.

By submitting the form you agree that you have answered all questions honestly, will inform a club coach if you test positive for COVID-19 within 48 hours of attending a class, and consent to club coaches confidentially notifying other club members on your session, should you test positive .

If you answer yes when asked if displaying any COVID-19 symptoms, we cannot permit you to participate in your class. You should follow Scottish Government Public Health guidelines.
